The Healthy Ageing Hub, locally managed by Kyeema Support Services, helps people across Glenelg Shire and western Victoria access the information, services and community connections they need to live well, stay independent and age well.
Open to people aged 50 years and over, and First Nations people aged 45 years and over, the Hub connects you with local health, wellbeing and community services, activities and support tailored to your needs. By making it easier to find the right information and opportunities to stay connected, the Healthy Ageing Hub supports active,healthy ageing, reduces social isolation and helps people continue living fulfilling, independent lives.
The program is funded by the Australian Government Department of Health, Disability and Ageing and commissioned by Western Victoria Primary Health Network (WVPHN).

- We're proud to work alongside local healthcare providers, neighbourhood houses, community organisations and support services throughout the Glenelg Shire. By building on the great work already happening across our region, we're creating a central service where people can easily discover local services, activities and community opportunities.
- Rather than replacing existing services, the Healthy Ageing Hub Kyeema strengthens the connections between them—making it easier for people to find the right support at the right time.

Do I need a referral to access the Heathy Ageing Hub Kyeema?
No. Just give us a call with any query you may have around being connected with services, groups and programs that assist with improving your wellbeing or lifestyle.
Do you provide direct service provision ?
No. We help link people in with already established providers and groups to assist in your goal to age well.
Do you provide crisis or emergency services or funding ?
No. We provide support by linking you with the correct providers, organisations and groups that can assist with your current circumstance of need.
Do you run any programs or activities?
Yes. When a possibility arises, we can facilitate opportunities to provide information sessions or hold activities in conjunction with existing established providers, educators, organisations and groups.

About Kyeema
Kyeema supports over 220 individuals and their families across the Glenelg Shire with One-on-One support 7 days a week in the community and at home, group based activities weekdays, after hours and weekends and employment support. All with an emphasis on health, well-being, fun and getting the most out of life.
